FORMS YOU NEED TO REGISTER FOR 2010 - 2011 TEAMS:

1) Registration Form

2) Payment Form

3) PCE Medical Waiver

4) Champions Gymnastics Club Waiver

** Fill out the above 4 forms and submit them either by:

a) Dropping them off at Champions Gymnastics Club (9527 - 49 St) to the PCE mailbox (on the Recreational Side of the gym)

b) Mailing them to PO Box 22598, Southbrook PO, Edmonton AB, T6W 0C3

c) Scan and email them to kim@powercheeredmonton.com (only if paying by credit card for the registration fee)
